New Student Information

Welcome! All new students are required to have up-to-date immunizations and a NJ physical on file. Please provide the immunization record and physical exam to the health office. We will also need your child's original health record. 

 

**Out of state/country students will have 30 days to provide the appropriate paperwork. In-state transfers should provide the appropriate paperwork upon beginning school**
