Sports Physical Information
Attention Parents/Guardians:
Please make sure the physician writes the actual date of the physical exam. The Physical Evaluation Form and the Clearance Form must be signed by the physician and must be complete, including height, weight, vision, pulse, and blood pressure. All abnormal findings must be explained. Your physician MUST complete and sign off on the Cardiac Assessment Professional Development Module. The History Form must be signed by the parent and student. *Please explain all “yes” answers; include dates, length of symptoms and treatment.
Attention Students:
It is your responsibility to check with the health office to determine if your physical is valid. If you wear glasses or contacts, please remember to bring them with you to your physical examination.
As it takes time to review and get sports physicals cleared by our physician, SPORTS PHYSICALS THAT ARE SUBMITTED AFTER THE DEADLINE MAY RESULT IN A DELAY IN YOUR PARTICIPATION. PLEASE ENSURE PHYSICALS ARE SUBMITTED BY THE DEADLINE! If you are unsure whether or not you will be participating, it is always safer to have the sports physical completed just in case. This way, if you decide you want to participate last minute, there will not be a delay.
REMINDER: The Scholastic Student-Athlete Safety Act (N.J.S.A. 18A:40-41.7) mandates that sports physicals may only be completed by a licensed physician, advanced practice nurse or physician’s assistant who has completed the Student-Athlete Cardiac Assessment professional development module. It is recommended that you verify that your medical provider has completed this module before conducting a student’s sports physical.

To participate in a fall sport, you must have a physical on file with the school nurse by Thursday, August 24, 2023.
School sports are offered during the fall, winter and spring seasons.
- Fall sports include: Boys’ Soccer, Girls’ Soccer, Girls’ Field Hockey, and Cross Country
- Winter sports include: Boys’ Basketball, Girls’ Basketball, Wrestling, and Cheerleading
- Spring sports include: Boys’ Baseball, Girls’ Softball, and Track and Field
Late submission of Sports Physical Forms may result in delay or ineligibility to participate.
The student’s parent must also complete the district Medical Update forms for participation prior to each sport season in which the student wishes to participate. These forms will be sent out through school messenger two weeks prior to each season.
