Sports Physical Information

Attention Parents/Guardians:

Please make sure the physician writes the actual date of the physical exam. The Physical Evaluation Form and the Clearance Form must be signed by the physician and must be complete, including height, weight, vision, pulse, and blood pressure. All abnormal findings must be explained. Your physician MUST complete and sign off on the Cardiac Assessment Professional Development Module. The History Form must be signed by the parent and student. *Please explain all “yes” answers; include dates, length of symptoms and treatment. 


Attention Students:

It is your responsibility to check with the health office to determine if your physical is valid. If you wear glasses or contacts, please remember to bring them with you to your physical examination.

 

As it takes time to review and get sports physicals cleared by our physician, SPORTS PHYSICALS THAT ARE SUBMITTED AFTER THE DEADLINE MAY RESULT IN A DELAY IN YOUR PARTICIPATION. PLEASE ENSURE PHYSICALS ARE SUBMITTED BY THE DEADLINE! If you are unsure whether or not you will be participating, it is always safer to have the sports physical completed just in case. This way, if you decide you want to participate last minute, there will not be a delay. 

 

REMINDER: The Scholastic Student-Athlete Safety Act (N.J.S.A. 18A:40-41.7) mandates that sports physicals may only be completed by a licensed physician, advanced practice nurse or physician’s assistant who has completed the Student-Athlete Cardiac Assessment professional development module. It is recommended that you verify that your medical provider has completed this module before conducting a student’s sports physical.

SPORTS PHYSICALS-
Pre-participation physical exams are valid for 365 days from the date of exam, and must fall on or within the first day of sports beginning. (i.e. sports begin September 10th 2023. Your sports physical exam must be dated ON or AFTER September 10, 2022, to be valid for that sports season).
 
Submit a paper copy only! No email copies will be accepted!
 
The packet must be completed in its ENTIRETY. Any incomplete packet will not be accepted and will be returned to you for completion.
 
On the history form (completed by the parent and reviewed by your physician), please be sure all yes answers on the history are  explained with ages and dates! Attach all clearances for injuries/surgeries from the last year and for any chronic medical conditions (i.e. cardiac conditions).

HEALTH HISTORY UPDATE QUESTIONNAIRE- 
Please note that with each season (Fall, Winter, Spring), a HHUQ will be required for participation in any sport. This form will be sent to your email via school messenger a few weeks prior to the beginning of sports. This form is necessary, in addition to the sports physical packet, for sports clearance. 
 
The HHUQ will act as a "Sign Up" for your child, in addition to providing updated medical and emergency contact information to the nurse and coach. 
APPROVAL FOR PARTICIPATION IN SPORTS-
The district physician will review your child's sports physical exam and either approve or deny clearance for sports. You will receive written notification of this.
 
All necessary information (as mentioned above) must be on file with the health office before the sports physical can be sent to the district physician for approval. This information will be considered in the determination of clearance by the district physician.

Notice Regarding Physical Examinations
To participate in a fall sport, you must have a physical on file with the school nurse by Thursday, August 24, 2023.

School sports are offered during the fall, winter and spring seasons.
  • Fall sports include: Boys’ Soccer, Girls’ Soccer, Girls’ Field Hockey, and Cross Country
  • Winter sports include: Boys’ Basketball, Girls’ Basketball, Wrestling, and Cheerleading
  • Spring sports include: Boys’ Baseball, Girls’ Softball, and Track and Field
 
In order to participate in any sport, the student must have a current Sports Physical completed by their physician on the approved NJ State form, and on file with the nurse. If any portion of the history and physical form is incomplete, it will be returned as required by state law. The completed sports physical is then reviewed and signed by the school physician.

Late submission of Sports Physical Forms may result in delay or ineligibility to participate.

The student’s parent must also complete the district Medical Update forms for participation prior to each sport season in which the student wishes to participate. These forms will be sent out through school messenger two weeks prior to each season.
