Communication of Medical and Emergency Information
EMERGENCY CONTACT INFORMATION
It is important that your child's emergency contact information is current. Please notify us immediately if there are any changes in telephone numbers or emergency contact persons. Changes can be reported to the main office. Your child cannot be released to any person not indicated as an emergency contact.
HEALTH INFORMATION
Alert the school nurse regarding any new information about the following. Information will remain confidential and will only be shared on an as needed basis with your child's teacher(s):
- Allergies
- Injuries
- Illness
- Medications
- Special accommodations
- Other medical issues concerning your child
