Requirements for extra-curricular activities and field trips

For safety and liability, your child will need to have all appropriate paperwork on file for participation in any extra-curricular activities (clubs, sports) and field trips. 

If your child has any medical conditions, the associated medication paperwork must be on file and the medication must be in the health office, with the exception of students who self-carry an emergency epinephrine or an inhaler. 
*If you feel your child does not require medication in school for their condition, please provide a note from the doctor indicating that the medication is not needed in school.*
 
If your child has any medical conditions that require additional clearance (i.e. cardiac conditions, injuries, etc), please ensure that a one-time clearance is on file with the health office. If there are any changes to the condition(s), please update the health office. 
 
**STUDENTS MUST HAVE MEDICATION ON THEM FOR ALL FIELD TRIPS**
If the medication is not kept in the health office (i.e epi pen or inhaler), your child is responsible to ensure they have the medication on their person each day. Students missing medication or associated medical paperwork cannot be sent on a class trip or off of school grounds.
